Parental Consent

All parents have been informed of their responsibilities and have agreed to their child’s participation in the exchange program. I have obtained and will retain on file with the CSFEP the necessary documents for the exchange, namely – the duly signed Participant application form (CSFEP 6); Medical form (CSFEP 7), as well as the Parental consent forms, and liability waivers (CSFEP 8) for all participants.
Participants

All participants have agreed to observe the code of conduct that we have established and understand the consequences of non-compliance.
Host Family Screening

All participants understand that they are involved in a reciprocal home-stay exchange. Host families have participated in the host screening procedures established locally for this program and have agreed to host the exchange participant with whom they have been twinned. Insurance requirements, liability issues and emergency procedures have been discussed.
a) At least two Family-Reference checks must be completed for each family wishing to host. A police check or a criminal and sex offender background check could also be done, to your own discretion. If you decide on a police check, kindly inform the CSFEP.

I accept the role and the responsibilities of group organizer with regard to this program which I planned and organized following the policies and procedures of School or Association and according to the guidelines for Canada Sports Friendship Exchange Program and Youth Exchanges Canada Sports and Group Program.

CSFEP 10 - EXCHANGE PREPARATION

1: Please describe in detail what pre-exchange activities is taking place or has taken place in order to prepare for the exchange? Which of these activities are the participants currently involved in or have already been involved in. e.g.: was any research done on your community or your host community; what fundraising activities are the participants doing, etc.?

2: What communication has taken place/is taking place between participants of both groups?  Which means of communication are participants using (i.e.: letters, telephone, e-mails, video, conferences, etc.)?

3: Before the exchange, how are you promoting Exchanges Canada, CSFEP and your exchanges in your local media? Please send us newspaper clippings, photos, or CD's etc., to add to your files. All these will be submitted to the Government.

CSFEP 11 - OBJECTIVES FOR CSFEP EXCHANGES

  • Objective 1)              To help young Canadians learn more about Canada.
  • Objective 2)              To help young Canadians connect to one another and form lasting friendships.
  • Objective 3)              To help young Canadians appreciate the diversity and shared aspects of the Canadian experience.

Please provide your detailed hosting plan of activities and indicate which activities you are

planning that would fulfil CSFEP’s 3 objectives.

Sample Itinerary below (This is just an example – feel free to use your preferred format)

Day 1:           

  • Visiting team departs for host region (bus, train, or plane)
  • Visiting team arrives - host team provides welcome lunch or dinner
  • Visiting team is billeted as per CSFEP matching criteria
  • Visiting players leave with host to the host’s home

(Depending on arrival time, other activities may be planned)

Day 2:           

  • Visiting players attend host school with host (flexible). 
  • Visiting players and host team visit the host area and historical sites in afternoon. (Flexible) 
  • Visiting players return to host homes for supper (flexible) 
  • Visiting players and host team play friendly game & exchange gifts related to the area of the province they are from if possible.

Day 3:            Visiting team participates in a cultural event or attends a friendly game.

Day 4:            Visiting team and host team visit other historical sites, geographical landmarks or museums of the host area.  (Example: Calgary host - visit to Banff National Park, Quebec, or Ontario host, visit Canada’s Parliament, Sugar Cabin, etc.)

Day 5:            Visit to Major Corporation in host area - meet President or CEO of company, have lunch at company cafeteria. After lunch, the visiting and host teams will attend a drug and alcohol prevention seminar conducted by a member of the RCMP Drug Awareness Program.

Day 6:            The visiting and host teams visit a pro or junior practice in visiting area, meet players, take pictures, collect autographs, etc., and then play friendly game in p.m.  Return to billet’s home for supper.

Day 7:            The visiting and host teams attend a farewell breakfast.  The visiting   team then leaves to go to the airport, to the train station or the bus departs for home.  Depending on departure time, other activities will be planned.
